چکیده

Computer assisted orthopaedic surgery (CAOS) was developed to improve the accuracy of surgical procedures, and it has dramatically improved in the last years from an experimental laboratory procedure to a routine procedure theoretically available for every orthopaedic surgeon. Besides robotic application, surgical navigation has been widely developed for joint arthroplasties, mostly for total knee replacement (TKR) and total hip replacement (THR). The ! rst navigated joint replacement (a TKR) was performed in January 1997 by Saragaglia et al (1), using a non-image based system with intra-operative kinematic and anatomic analysis of the knee joint. Following these pioneers, other systems have been developed and validated. However, this appealing technology has not been adopted on a broad scale, and some surgeons complain of several relevant inconveniencies of using it on a routine basis. What have we already learnt, and what are the remaining questions?

Consent process for elective total hip and knee arthroplasty.

PURPOSE To assess the consent process for elective primary total hip replacement (THR) and total knee replacement (TKR) in our hospital. METHODS Consent processes of 47 THR and 53 TKR patients performed by 11 surgeons were reviewed. Complications that were documented were recorded, as was the grade of surgeon (consultant or specialist trainee) performing consent, and the location at which thi...

Arthroscopic Partial Meniscectomy for Painful Degenerative Meniscal Tears in the Presence of Knee Osteoarthritis in Patients Older than 50 Years of Age: Predictors of an Early (1 to 5 Years) Total Knee Replacement

Background: The role of arthroscopic partial meniscectomy (APM) for painful degenerative meniscal tears (PDMT) is currently controversial. To define the rate of early (1 to 5 years) conversion to total knee replacement (TKR) and their predictors after APM for PDMT in patients with knee osteoarthritis and more than 50 years of age. Methods: Retrospective cohort study of patients more than...

Insufficient duration of venous thromboembolism prophylaxis after total hip or knee replacement when compared with the time course of thromboembolic events: findings from the Global Orthopaedic Registry.

Patients who have undergone total hip or knee replacement (THR and TKR, respectively) are at high risk of venous thromboembolism. We aimed to determine the time courses of both the incidence of venous thromboembolism and effective prophylaxis. Patients with elective primary THR and TKR were enrolled in the multi-national Global Orthopaedic Registry. Data on the incidence of venous thromboemboli...

Lower limb orthopaedic surgery results in changes to coagulation and non-specific inflammatory biomarkers, including selective clinical outcome measures

BACKGROUND With an aging society and raised expectations, joint replacement surgery is likely to increase significantly in the future. The development of postoperative complications following joint replacement surgery (for example, infection, systemic inflammatory response syndrome and deep vein thrombosis) is also likely to increase. Despite considerable progress in orthopaedic surgery, compar...

Novel oral anticoagulants for venous thromboembolism prophylaxis after total hip or knee replacement: an update on rivaroxaban (xarelto).

IntroductIon In 2005, approximately 285,000 individuals underwent total hip replacement (THR) procedures, and approximately 523,000 individuals had total knee replacement (TKR) procedures in the U.S.1 As the population ages, it is estimated that by 2030 the demand for these surgeries will increase, and the numbers are expected to grow to 572,000 THR procedures and 3,480,000 TKR procedures annua...
